If you are a Wisconsin resident, please fill out the form on the menu above. It is listed under either ART FAIR/Application(s) for Exhibitors/Request Applications. You may select applications for either or both art fairs. Be sure to fill in the form one time only. In that way you may receive paper applications or notification when they are ready to be downloaded. Your name and address will remain on our mailing list until an application is returned by the U.S. Postal Service or you notify us that you are no longer interested in receiving them. Thank you.
